Position Summary

The Temporary Case Manager will play a pivotal role in delivering exceptional clinical and case management support to ensure clients receive superior care that aligns with established standards. This role is crucial for maintaining high-quality direct service delivery in both individual and group settings.

This position is designed for a temporary engagement, aimed at covering for an employee on leave. It entails a full-time commitment of 40 hours per week.

Key Responsibilities
  • Deliver personalized case management and outreach tailored to patient needs.
  • Collaborate with patients and the team to assess needs and formulate treatment plans.
  • Foster trusting relationships with clients, emphasizing independence and improved health outcomes.
  • Support patients in daily activities, appointments, budgeting, and community integration.
  • Maintain accurate and up-to-date patient records in compliance with organizational standards.
  • Provide regular updates on case progress as required.
  • Contribute to quality improvement initiatives as part of the organization’s programming.
  • Ensure mental health services reflect a model focused on client choice and rehabilitation.
  • Work collaboratively with multidisciplinary teams and participate in staff meetings and training sessions.
  • Conduct assessments and documentation in a timely manner, adhering to industry standards.
  • Meet productivity targets for direct service provision.
  • Adhere to all health and safety protocols.
  • Comply with organizational policies and procedures.
  • Engage proactively with colleagues and management.

Qualifications

Education: High school diploma and an Associate's degree in social services required; Bachelor's degree preferred.

Experience: Minimum of three years working with diverse populations; healthcare experience is a plus.

Skills: Proficient in documentation and reporting; conflict resolution skills are essential.

Licenses/Certifications: Valid California Driver's License and CPR/First Aid certification required.

Language Proficiency: Strong command of English; bilingual Spanish skills are advantageous.

Technical Skills: Proficient in MS Office Suite and internet applications.
