Job Summary

The Staff Pad is seeking a highly motivated and compassionate Nurse Navigator to join our team. As a key member of our oncology care team, you will play a vital role in providing exceptional patient care and support to individuals and their families affected by cancer.

Key Responsibilities
  • Assess the physical and psychosocial needs of patients and their families, providing personalized support and guidance throughout their cancer journey.
  • Meet with patients in various settings, including hospitals, clinics, and community centers, to provide consultation, recording, and summary services.
  • Act as a patient advocate, facilitating effective communication between patients, healthcare providers, and other stakeholders to ensure seamless care coordination.
  • Develop and implement up-to-date care plans in collaboration with multidisciplinary teams, ensuring timely and high-quality care.
  • Provide emotional support and nursing education to patients and their families, empowering them to make informed decisions about their care.
  • Document patient interactions, correspondence, and care in electronic medical records, ensuring accurate and comprehensive documentation.
  • Coordinate referrals to specialists, prepare correspondence, and communicate with referring agencies and primary care providers to enhance continuity of care.
  • Link patients and their families to relevant national, community, hospital, and medical resources, including financial counseling, social work, nutrition, rehabilitation, psychiatric, palliative care, and end-of-life care.
  • Assist in case selection, coordinate, and prepare weekly tumor board presentations, leveraging expertise to identify and implement improvement processes.
  • Collaborate with marketing and outreach departments to educate the public and referring physicians about services, and assist Public Relations in providing cancer prevention and early detection programs.
  • Maintain a personal knowledge base of available resources, both in-house and outside, and increase knowledge of cancer-related terms, common procedures, and up-to-date treatment protocols.
Requirements
  • Bachelor's degree in nursing or a related field.
  • Montana Registered Nurses license.
  • At least three years' experience in nursing.
  • OCN certification within two years of hire for NN working with all tumor sites.

Preferred Qualifications: Master's degree in nursing or a related field, certification in oncology, and experience working with lung, genitourinary, and breast cancer patients.
