Job Summary

The Staff Pad is seeking a highly skilled and compassionate Nurse Navigator to join our team. As a key member of our oncology care team, you will play a vital role in providing exceptional patient care and support to individuals and their families affected by cancer.

Key Responsibilities
  • Assess the physical and psychosocial needs of patients and their families, providing personalized care and support throughout their cancer journey.
  • Meet with patients in various settings, including clinics, hospitals, and community centers, to provide consultation, recording, and summary services.
  • Act as a patient advocate, facilitating effective communication between patients, families, and healthcare providers to ensure seamless care coordination.
  • Develop and implement up-to-date care plans in collaboration with multidisciplinary teams, prioritizing patient-centered care and outcomes.
  • Provide emotional support and nursing education to patients and their families, empowering them to make informed decisions about their care.
  • Document patient interactions and care plans in electronic medical records, ensuring accurate and timely information sharing.
  • Coordinate referrals to specialists, communicate with healthcare teams, and prepare correspondence to enhance continuity of care.
  • Link patients and families to community resources, including financial counseling, social work, nutrition, rehabilitation, and palliative care services.
  • Assist in case selection, coordinate tumor board presentations, and identify opportunities for process improvements.
  • Collaborate with marketing and outreach departments to educate the public and referring physicians about our services and cancer prevention programs.
Requirements
  • Bachelor's degree in nursing or a related field.
  • Current Montana Registered Nurse license.
  • At least three years of nursing experience.
  • OCN certification within two years of hire for NN working with all tumor sites.
