Job Title: Hospice Social Worker

At Compassus, we are seeking a compassionate and skilled Hospice Social Worker to join our team. As a key member of our interdisciplinary team, you will provide essential support to patients and their families, helping them navigate the complexities of hospice care.

Job Summary:

The Hospice Social Worker will be responsible for:

  • Providing preventive, educational, and evaluative services to meet the psychosocial needs of patients and their families
  • Utilizing community resources and the interdisciplinary team to aid in the care process
  • Modeling Compassus values and promoting the Compassus philosophy
  • Upholding the Code of Ethical Conduct and promoting positive working relationships
Responsibilities:

The successful candidate will be responsible for:

  • Explaining hospice services and Medicare benefits to patients and families
  • Obtaining informed consent and election of benefits documents
  • Submitting documentation and paperwork to facilities in a timely manner
  • Participating in the interdisciplinary team and developing care plans
  • Providing psycho-social support and counseling services to patients and families
  • Collaborating with the interdisciplinary team to coordinate care and support
Requirements:

To be successful in this role, you will need:

  • A minimum of a Bachelor's degree in Social Work, Psychology, Sociology, or a related field
  • One year of social work experience in a healthcare setting
  • Licensure, if required by state rules
Skills and Abilities:

The ideal candidate will possess:

  • Strong written and verbal communication skills
  • Ability to read, analyze, and interpret business and technical procedures
  • Ability to effectively present information and respond to questions
  • Strong problem-solving and critical thinking skills
Work Environment:

This role requires regular standing, walking, and manual dexterity, as well as the ability to lift and move objects up to 50 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus.
