POSITION SUMMARY

The Permanent Supportive Housing Case Manager plays a crucial role in delivering comprehensive and intensive case management services to individuals experiencing chronic homelessness. This position is dedicated to fostering trusting relationships with clients, empowering them to maintain stable housing and achieve their personal objectives. The Case Manager employs evidence-based methodologies, including a Housing First approach, Motivational Interviewing, Harm Reduction, and Trauma-Informed Care.

The Case Manager collaborates with various service providers to support clients in achieving housing stability, enhancing financial independence, accessing wellness resources, and promoting community integration. The ultimate aim is to maximize housing retention and implement strategies to prevent recidivism into homelessness.

KEY RESPONSIBILITIES

  • Deliver on-site case management services that promote client self-sufficiency and housing retention.
  • Establish positive and trusting relationships with diverse populations, particularly those facing trauma, homelessness, and health challenges.
  • Utilize evidence-based practices in service delivery, including Trauma-Informed Care, Motivational Interviewing, Harm Reduction, and Person-Centered Planning.
  • Provide site-based case management in areas such as independent living skills, community integration, access to benefits, employment services, and healthcare.
  • Manage a caseload of 12-13 clients, conducting weekly meetings to monitor progress.
  • Assist clients in creating Individualized Service Plans, ensuring their active participation in the process.
  • Conduct crisis assessments and implement appropriate interventions.
  • Collaborate with community partners to address any issues affecting client housing stability.
  • Maintain accurate documentation and data entry in the Homeless Management Information System (HMIS).
  • Organize community-building activities to recognize client achievements and foster leadership.
  • Model effective conflict resolution techniques to mediate landlord-tenant relationships.
  • Facilitate the orientation of new clients and ensure smooth transitions for departing clients.
  • Identify opportunities for volunteer involvement.
  • Perform additional duties as assigned.

QUALIFICATIONS

Successful candidates will possess the ability to fulfill essential duties effectively. Previous experience in case management and familiarity with local resources are highly desirable. Strong time management skills and effective communication abilities are essential. A valid driver's license and reliable transportation are required.

EDUCATION AND EXPERIENCE

A bachelor's degree is required, preferably in social work, human services, or a related field. A minimum of two years of case management experience and familiarity with HMIS is preferred. Extensive experience working with at-risk populations and crisis intervention is essential.

LANGUAGE SKILLS

Bilingual proficiency in English and Spanish is required.

PHYSICAL DEMANDS

This position requires the ability to sit for extended periods, use computer equipment, operate a vehicle, and perform physical tasks such as walking and lifting up to 20 lbs. Reasonable accommodations may be made for individuals with disabilities.

WORK ENVIRONMENT

The work environment is primarily indoors, with occasional outdoor activities for community events. Some weekend and evening hours may be required. The noise level is typically moderate.
