SUMMARY

The Permanent Supportive Housing Case Manager is a skilled professional in social services, tasked with delivering comprehensive and intensive case management to individuals facing chronic homelessness within the community. This role focuses on establishing trust with participants, facilitating their ability to maintain housing and achieve personal objectives. The Case Manager is committed to empowering participants through evidence-based methodologies, including a Housing First approach, Motivational Interviewing, Harm Reduction, and Trauma-Informed Care.

The Case Manager collaborates with various service providers to assist participants in achieving housing stability, enhancing financial independence, connecting to wellness resources, and fostering community integration. Supportive services are tailored to meet the unique needs of participants, particularly in areas such as physical and mental health disabilities, domestic violence, and substance use. The ultimate aim is to ensure housing stability and implement strategies to prevent participants from returning to homelessness.

KEY RESPONSIBILITIES

  • Deliver on-site case management services that promote client self-sufficiency and housing retention.
  • Establish positive and trusting relationships with diverse populations, especially those experiencing trauma, homelessness, and various health challenges.
  • Utilize evidence-based practices in service delivery, including Trauma-Informed Care, Motivational Interviewing, Harm Reduction, and Person-Centered Planning.
  • Provide site-based case management in areas such as independent living skills, community integration, access to mainstream benefits, employment services, and healthcare.
  • Manage a caseload of 12-13 individuals, meeting with each participant weekly.
  • Assist participants in creating Individualized Service Plans, ensuring their active involvement in the design and delivery of services.
  • Conduct crisis assessments and implement appropriate interventions.
  • Collaborate with community partners to address any issues that may affect participants' housing stability.
  • Maintain accurate documentation and statistics in the Homeless Management Information System (HMIS) as per established guidelines.
  • Organize community-building activities to recognize participant achievements and encourage leadership.
  • Model effective conflict resolution techniques to mediate landlord-participant relationships.
  • Facilitate transitions for new and departing clients, ensuring continuity of care.
  • Identify opportunities for volunteer involvement.
  • Perform additional duties as assigned.

QUALIFICATIONS

To excel in this role, candidates must demonstrate proficiency in the essential duties outlined. Previous experience in case management and familiarity with local resources and social services are highly valued. Strong time management skills and effective communication abilities, both verbal and written, are essential. A valid driver's license and reliable transportation are required, along with liability insurance. Candidates should possess a high level of energy, creativity, and adaptability.

EDUCATION and EXPERIENCE

A bachelor's degree is required, preferably in social work, human services, or a related field. A minimum of two years of case management experience and familiarity with HMIS is preferred. Extensive experience working with at-risk populations and crisis intervention is essential.

LANGUAGE SKILLS

Bilingual proficiency in English and Spanish is required.

MATHEMATICAL SKILLS

Basic mathematical skills are necessary for fulfilling job responsibilities, including the ability to perform calculations and manage budgets.

REASONING ABILITY

Case managers must be attuned to the complexities associated with homelessness and at-risk populations, demonstrating sound judgment and crisis management skills.

CERTIFICATES, LICENSES, REGISTRATIONS

A Class C driver's license is required. Licensure as a social worker or equivalent is desirable.

PHYSICAL DEMANDS

The physical demands of this position include prolonged sitting, computer use, vehicle operation, walking, climbing stairs, bending, stretching, and lifting up to 20 lbs. Reasonable accommodations may be made for individuals with disabilities.

WORK ENVIRONMENT

This role primarily involves indoor work, with occasional outdoor activities for community events. Some weekend and evening hours may be required. The work environment is typically moderate in noise level.
