Job Summary

The Baptist Medical Center is seeking a skilled Registered Nurse Case Manager to join our team. As a key member of our healthcare team, you will be responsible for coordinating patient care across the continuum, ensuring seamless transitions, and promoting optimal health outcomes.

Key Responsibilities
  • Facilitate care coordination to ensure patients receive the right care at the right time
  • Assess patients for transition needs and develop plans to prevent avoidable readmissions
  • Collaborate with healthcare teams to ensure efficient throughput and timely discharges
  • Ensure compliance with regulatory requirements, accreditation standards, and organizational policies
  • Provide education to patients, families, and caregivers on healthcare management and self-care
Requirements
  • Graduate of an accredited School of Nursing
  • 2 years of acute hospital patient care experience, with utilization/case management experience
  • Current Texas State license for Registered Nurse
  • Accredited Case Management (ACM) certification preferred
