Resource Patient Care Coordinator

Shodair is seeking a highly motivated and compassionate Resource Patient Care Coordinator to join our Case Management team. As a vital member of our interdisciplinary care team, you will work closely with patients, families, and healthcare professionals to develop and implement individualized care plans.
The ideal candidate will possess excellent interpersonal and customer service skills, with a passion for mental health and advocating for patients. You will be responsible for providing a safe and welcoming environment for our new patients, while also ensuring seamless transitions between treatment and admissions teams.

RequirementsRequired Education and Experience:
- At least one year of experience serving people with mental illness
Preferred Education and Experience:
- Two years of case management experience or patient care experience in a healthcare or social services delivery system
- Post-secondary education or Bachelor's degree in a human services field
