Job Summary

We are seeking a highly skilled and experienced Clinical Manager to join our team at Eastern Connecticut Health Network (ECHN). The successful candidate will be responsible for managing patient care services in a home health agency setting, ensuring high-quality care and exceptional patient outcomes.

Key Responsibilities
  • Manage professional and paraprofessional nursing services, including those provided by contract to hospice patient families.
  • Assess and evaluate community needs, agency needs, and agency resources to inform staffing decisions and recommendations.
  • Ensure clinical competency of staff assigned and provide ongoing training and development opportunities.
  • Conduct regular performance evaluations and provide feedback to staff.
  • Collaborate with interdisciplinary teams to develop and implement patient care plans.
  • Ensure compliance with regulatory requirements and agency quality assurance goals.
  • Participate in hiring, interviewing, orienting, and terminating staff as needed.
  • Facilitate open communication between staff and senior management.
Requirements
  • Registered Nurse with an active license to practice nursing in Connecticut.
  • Master's degree from a program approved by the National League for Nursing or the American Public Health Association, or a baccalaureate degree in nursing with a minimum of 3 years of full-time clinical experience in nursing.
  • Experience in home health agency or hospice care preferred.
  • Strong leadership and management skills, with the ability to motivate and develop staff.
  • Excellent communication and interpersonal skills.
  • Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities.
