Key Responsibilities

The Hospice Nurse Practitioner is responsible for providing expert consultation in palliative care, symptom management, and supportive services tailored to the needs of hospice patients as directed by the referring physician.
  • Conducts in-person visits to eligible hospice patients within designated timeframes and communicates clinical findings to the Hospice Medical Director.
  • Collaborates with the Hospice Medical Director as necessary, informs the primary physician of recommended services, and works alongside other healthcare providers as needed.
  • Develops and manages the care plan, documenting issues, goals, interventions, and responses from patients and families regarding hospice care.
  • Educates and guides patients and families in self-care practices when appropriate.
  • Keeps precise and relevant clinical records concerning the patient's status, reporting any changes or needs to the Administrator and Hospice Medical Director.

Qualifications

License Requirements:
  • Current CPR certification is mandatory.
  • Valid certification as a clinical Nurse Specialist or Nurse Practitioner in the state of practice.
  • Certification from ANCC or AANP is required.
  • Possession of a valid driver's license, vehicle insurance, and access to reliable transportation.
  • NPI Number and DEA with prescribing privileges are essential.
  • PECOS enrollment certification and CAQH enrollment are required.
  • Medicare and Medicaid numbers are preferred but not mandatory.

Experience Requirements

  • A minimum of one year of experience as a Nurse Practitioner in chronic disease management, family, or primary care is preferred.
