Key Responsibilities

The Hospice Nurse Practitioner plays a vital role in providing expert consultation in palliative care, symptom relief, and supportive services tailored to the needs of hospice patients, as directed by the referring physician.
  • Conducts in-person visits to eligible hospice patients within designated timeframes and communicates clinical findings to the Hospice Medical Director.
  • Collaborates with the Hospice Medical Director as necessary, updates the primary physician on recommended services, and works alongside other healthcare providers as needed.
  • Develops and coordinates individualized care plans, documenting issues, objectives, interventions, and patient/family responses to hospice services.
  • Educates and guides patients and families in self-care practices when suitable.
  • Keeps precise and relevant clinical records regarding the patient's status, reporting any changes or concerns to the Administrator and Hospice Medical Director.
Qualifications

License Requirements:
  • Current CPR certification is mandatory.
  • Active certification as a Nurse Practitioner or Clinical Nurse Specialist in the state of practice.
  • Certification from ANCC or AANP is required.
  • Possession of a valid driver's license, vehicle insurance, and access to reliable transportation.
  • NPI Number.
  • DEA registration with prescribing authority.
  • PECOS enrollment certification.
  • CAQH enrollment.
  • Medicare and Medicaid numbers (the latter is preferred but not mandatory).
Experience Requirements

  • Minimum of one year of experience as a Nurse Practitioner in chronic disease management, family, or primary care settings.
