Job Summary:

As a Hospice Nurse Practitioner with Concho Hearts Hospice, you will play a vital role in providing compassionate care to patients and their families. Your primary functions will include conducting clinical examinations, performing diagnoses, and coordinating care with our interdisciplinary team.

Responsibilities:
  • Under the supervision of a physician, you will visit patients, complete physical assessments, and evaluate hospice eligibility in consultation with our team.
  • You will provide medical care, palliate, and manage terminal illnesses in compliance with the state Nurse Practice Act.
  • Key responsibilities include:
    • Completing history and physical examinations to evaluate patients' medical conditions.
    • Managing health conditions and terminal illnesses.
    • Completing hospice face-to-face visit requirements.
    • Providing clinical information to our medical director for interpretation of eligibility for hospice recertification.
Qualifications:
  • Education/Licensure: You must be a licensed Nurse Practitioner with the State Nurse Practice Act and licensed in the state where our branch is located.
Experience:
  • Minimum of one year of Nurse Practitioner experience is required.
  • We prefer a minimum of six months to one year of experience in hospice or pain and symptom management.
Skills:
  • Nursing skills as defined by generally accepted standards of practice.
  • Excellent interpersonal skills and the ability to communicate effectively with all segments of our staff.
  • Demonstrated decision-making skills.
  • Proof of current CPR.
Transportation:
  • Reliable transportation.
  • Valid and current auto liability insurance.
Environmental and Working Conditions:

You will work in patients' homes in various conditions, with possible exposure to blood and bodily fluids and infectious diseases. You must be able to work a flexible schedule, travel locally, and tolerate some exposure to unpleasant weather. You may be required to work on PRN emergency calls.

Physical and Mental Effort:

Prolonged standing and walking are required, with the ability to lift up to 50 lbs and move patients. You must be able to work under some stressful conditions to meet deadlines and patient needs, and make quick decisions and resource acquisition to meet patient/family individualized psycho-social needs.
