Position Overview:
The primary responsibilities of the Nurse Practitioner include conducting clinical evaluations of patients, diagnosing based on assessments, and coordinating care with the interdisciplinary team.

Key Responsibilities:
  • Under physician supervision, the Nurse Practitioner visits patients, performs comprehensive physical assessments, and evaluates hospice eligibility in collaboration with the hospice interdisciplinary team.
  • Delivers medical care, alleviates symptoms, and manages terminal illnesses in accordance with the state Nurse Practice Act.
  • Conducts thorough history and physical examinations to assess the patient's medical condition.
  • Oversees health conditions and terminal illnesses.
  • Completes the necessary hospice face-to-face visit requirements.
  • Provides clinical data to the hospice medical director for evaluation regarding eligibility for hospice recertification.

Qualifications:
  • Education/Licensure: Must be a licensed Nurse Practitioner in compliance with the State Nurse Practice Act in the state of assignment.

Experience:
  • At least one year of experience as a Nurse Practitioner is required.
  • Preferred experience includes a minimum of six months to one year in hospice care or pain and symptom management.

Skills:
  • Demonstrated nursing skills consistent with generally accepted standards of practice.
  • Exceptional interpersonal skills with the ability to communicate effectively across all levels of staff.
  • Proven decision-making capabilities.
  • Current CPR certification is required.

Transportation Requirements:
  • Must have reliable transportation.
  • Valid and current auto liability insurance is necessary.

Work Environment: The role involves working in patients' homes under various conditions, with potential exposure to blood, bodily fluids, and infectious diseases. Flexibility in scheduling and local travel is required, along with some exposure to adverse weather conditions and PRN emergency calls.

Physical and Mental Demands: The position requires prolonged standing and walking, with the ability to lift up to 50 lbs and assist patients. The role may involve working under stressful conditions to meet deadlines and patient needs, necessitating quick decision-making and resource management to address individualized psycho-social needs.
