Full-Time Hospice Registered Nurse

Position Overview:

The Hospice RN is responsible for planning, organizing, and directing hospice services, emphasizing community health education and patient experience. In this role, you will apply your nursing expertise to deliver high-quality care, evaluate patient needs, formulate care plans, administer medications, and coordinate overall patient management. You will also collaborate closely with the interdisciplinary team to support the emotional health of patients and their families.

Key Responsibilities:

Patient Care
  • Conduct initial, comprehensive, and ongoing evaluations of patients and families to identify hospice requirements
  • Deliver professional nursing care utilizing the nursing process
  • Create care plans and set objectives based on nursing assessments
  • Administer prescribed medications and treatments
  • Assist patients with daily living activities and self-care
  • Serve as a Case Manager to oversee patient care
Communication
  • Maintain precise clinical documentation and records
  • Engage with physicians and community health professionals
  • Educate patients and families on self-care practices
  • Ensure a secure environment for patients
  • Collaborate with the interdisciplinary team to address emotional needs
Additional Responsibilities
  • Participate in on-call duties as required
  • Coordinate necessary items and services for patient care
  • Supervise ancillary staff
  • Enhance professional knowledge through ongoing education
  • Engage in quality assessment and performance improvement initiatives
Qualifications:
  • Graduate of a National League for Nursing accredited nursing program
  • At least two years of nursing experience, preferably in public health, home care, or hospice settings
  • Current licensure to practice professional nursing
  • Strong observational, problem-solving, and communication abilities
  • Capability to independently organize and prioritize workload
  • Current CPR certification
  • Valid driver's license and dependable transportation
