Job Summary

We are seeking a highly skilled and compassionate Social Worker MSW to join our team. As a Social Worker MSW, you will play a vital role in providing psychosocial support to patients and their families, working closely with our interdisciplinary team to develop and implement individualized care plans.

Key Responsibilities
  • Conduct comprehensive assessments of patients' psychosocial needs and develop individualized care plans to address these needs.
  • Provide emotional support and counseling to patients and their families, helping them to cope with the challenges of terminal illness and end-of-life care.
  • Collaborate with our interdisciplinary team to ensure that patients receive comprehensive and coordinated care.
  • Document patient progress and outcomes, using this information to inform care planning and improve patient outcomes.
  • Participate in ongoing education and training to stay up-to-date on best practices in hospice care and social work.
Requirements
  • Master's Degree in Social Work from a CSWE-accredited school.
  • Licensure as a Social Worker in the state where the hospice is located.
  • At least one year of experience as a Social Worker in a healthcare or hospice setting, with three to five years of experience preferred.
  • Knowledge of terminally ill patients and their families, as well as an understanding of hospice care and the psychosocial dynamics of illness, loss, and death.
