Key Responsibilities:
  1. Perform thorough, holistic assessments in clients' homes.

  4. Provide ongoing evaluations, oversight, and advocacy to ensure clients' needs are met.

  6. Engage with the Care Management team for professional growth and to enhance departmental processes.
Candidate Profile:
  1. Possession of a valid MSW, MFT, MA in Gerontology, or a related field is highly preferred (minimum two years of relevant experience).
  2. Comprehensive understanding of the aging process and care management best practices.
  3. Strong communication skills, both written and verbal, along with excellent attention to detail and time management.
  4. Demonstrated integrity, honesty, and commitment to high professional standards.
  5. Ability to collaborate effectively within a multidisciplinary team.
  6. Availability for weekdays and occasional evening/weekend on-call duties.
  7. Valid driver's license, insurance, and reliable transportation.

Compensation:
