Key Responsibilities:- Perform thorough, holistic assessments in clients' homes.
- Create and execute clear, goal-oriented care plans that cater to clients' health, functional, and quality of life requirements.
- Act as a liaison between families and professionals, effectively managing family dynamics to achieve client objectives.
- Provide ongoing evaluations, oversight, and advocacy to ensure clients' needs are met.
- Document services provided with detailed and timely reports.
- Engage with the Care Management team for professional growth and to enhance departmental processes.
- Possession of a valid MSW, MFT, MA in Gerontology, or a related field is highly preferred (minimum two years of relevant experience).
- Comprehensive understanding of the aging process and care management best practices.
- Strong communication skills, both written and verbal, along with excellent attention to detail and time management.
- Demonstrated integrity, honesty, and commitment to high professional standards.
- Ability to collaborate effectively within a multidisciplinary team.
- Availability for weekdays and occasional evening/weekend on-call duties.
- Valid driver's license, insurance, and reliable transportation.
