Position Overview

Qualifications for Home Health Occupational Therapist:

  • Current California Occupational Therapist (OT) license
  • CPR Certification
  • Dependable transportation and valid driver's license with proof of insurance required.

Key Responsibilities:

  • Facilitate discharge planning by collaborating with physicians, nurses, social workers, and other healthcare professionals.
  • Accurately document patient care services through timely charting.
  • Enhance patient independence by selecting and designing therapies tailored to their physical abilities and interests.
  • Assess the effectiveness of occupational therapy by observing, recording, and evaluating patient progress; suggest and implement necessary adjustments and modifications.
  • Execute physician-recommended occupational therapies for self-care tasks.
  • Assist patients in developing or regaining physical or mental capabilities, or adapting to their disabilities.
