Job Summary

The Admissions RN is responsible for providing high-quality patient care and ensuring a smooth transition for patients and their families. This role requires strong communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.

Responsibilities

  • Perform patient visits and assessments to determine the appropriate plan of care
  • Collaborate with the interdisciplinary team to ensure continuity of care
  • Develop and implement individualized care plans to meet the unique needs of each patient
  • Provide education and support to patients and their families
  • Assist with discharges from facilities to the site of hospice care
  • Perform admissions at the site of hospice care
  • Assist the hospital liaison team to meet the needs within the hospitals
  • Other hospice nursing visits as assigned

Qualifications

  • Associate degree in nursing with 1-2 years acute care experience required
  • Certification as hospice nurse preferred
  • 1-2 years hospice experience preferred
  • Current CA Registered Nurse (RN) professional license
  • Demonstrated ability to work effectively with patients, physicians, community agencies, and institutions
  • Proficient computer skills required
  • Demonstrated ability to multi-task and manage crisis situations
  • Current proof of CPR-BLS and negative TB test result
  • Current driver's license with proof of insurance
  • Bi-lingual (Spanish/English) preferred

The Elizabeth Hospice is San Diego's oldest non-profit hospice of 45 years. We are growing and looking for talented individuals to join our high-performing team and support our mission. We serve all of San Diego county and south Riverside county with multiple hospice office locations.

Work Schedule

12:00 p.m.-8:30 p.m. 5 days, days will vary.

Salary

$45-$60 Hourly. Subject to experience and qualifications.

Travel Required

Travel required for patient visits throughout San Diego and South Riverside County. Occasional travel for educational purposes.

Weekend Commitment

Every Other Weekend.
