The Elizabeth Hospice is seeking a skilled Licensed Vocational Nurse (LVN) to join our team in the Hospice Admissions department. As an LVN, you will play a vital role in providing professional nursing care to patients across the lifespan, under the supervision of a Registered Nurse (RN).
Responsibilities:- Assist RNs in analyzing situations, anticipating problem areas, and taking appropriate action.
- Evaluate patient responses to care and document outcomes in a timely and safe manner.
- Perform licensed nurse skills, including phlebotomy, as stated in the basic skills checklist and policies and procedures.
- Implement and document teaching plans based on patient and family needs, in a timely and culturally sensitive manner.
- Adhere to direction provided by RNs on patient care plans and deliver services under their supervision.
- Document all care provided to patients, including telephone interactions with other staff members, and follow charting standards.
- Enter or retrieve data from established computer files and orient, instruct, and train assigned personnel.
- Perform phlebotomy as needed and provide support to the agency in completing assigned audits and office duties.
- Minimum 12 months of experience as an LVN.
- Preferred experience of 1-2 years in an acute care setting, home health, or hospice.
- Current California LVN license.
- Current CPR certification and negative TB skin test.
- Ability to answer phones, maintain patient charts, and schedule appointments.
- Phlebotomy certification preferred.
- CHPLN certification preferred.
- Spanish speaking may be preferred or required.
- Current California driver's license and proof of auto insurance required.
