Registered Nurse Case Manager for Hospice Care

Position Overview

The Elizabeth Hospice is seeking a dedicated RN Case Manager to deliver exceptional nursing care to our patients, including adults, children, and perinatal cases. Collaborating with the interdisciplinary team (IDT), patients, and their families, you will assess, create, implement, and evaluate individualized care plans. Effective communication and coordination of care with all stakeholders are essential to this role, ensuring that our mission, vision, values, and commitment to quality are upheld.

Compensation: $45.00-$61.00 per hour, based on RN licensure experience.

Work Schedule: Monday to Friday, 8:00 a.m. to 5:00 p.m.

On-call responsibilities include 2 shifts per month, which may involve weekend duties.

Key Responsibilities
  1. Develop and document comprehensive care plans in collaboration with the interdisciplinary team.
  2. Oversee the execution of care plans, making adjustments as needed to meet the evolving needs of patients and families.
  3. Direct the care provided by LVNs and HAs, ensuring alignment with the established care plan.
  4. Supervise services delivered by LVNs and HAs in accordance with organizational policies and regulatory standards.
  5. Communicate any changes in patient status to the IDT and primary physician.
  6. Perform clinical duties as outlined in the competency checklist and adhere to established policies and standards of care.
  7. Maintain accurate and timely patient documentation through electronic charting.
  8. Implement and uphold hospice policies, procedures, and regulatory requirements.
  9. Provide health education to patients and their families.
  10. Orient and train new staff members as required.
  11. Participate in providing continuous coverage for patients and families, including on-call duties.
  12. Conduct admission assessments and facilitate admissions when necessary.
  13. Adhere to established standards in all job functions.
Qualifications
  1. Associate's Degree required; Bachelor's Degree preferred.
  2. A minimum of 12 months of nursing experience, with 1-2 years in acute care preferred.
  3. Current California RN license is mandatory.
  4. Valid California driver's license with proof of insurance is required.
  5. Certification as a hospice nurse is preferred.
  6. Pediatric experience is advantageous.
  7. Spanish language proficiency is preferred.
  8. Proficient in computer skills, including word processing and use of office equipment.
  9. Must be able to drive in various conditions and access homes with pets.
