Registered Nurse Case Manager for Hospice Care

The Elizabeth Hospice is seeking a dedicated RN Case Manager responsible for delivering high-quality nursing care to a diverse patient population, including adults, pediatrics, and perinatal cases. Collaborating with the Interdisciplinary Team (IDT), the RN will assess, formulate, implement, and review individualized care plans, ensuring effective communication and coordination of care among all stakeholders.
Compensation: $45.00-$61.00 per hour, based on RN licensure experience.
Work Schedule: Monday to Friday, 8:00 a.m. to 5:00 p.m.
On-call responsibilities include 2 shifts per month, potentially requiring weekend availability.
Location: Primarily serving the Escondido area and surrounding regions.
Travel is necessary for patient visits.
The Elizabeth Hospice, with 45 years of service, is the oldest non-profit hospice in San Diego, committed to enhancing the quality of life for individuals nearing the end of their journey and supporting their families.
Key Responsibilities
- Develop and maintain comprehensive care plans in collaboration with the IDT.
- Oversee the execution of care plans, making adjustments as needed to address patient and family requirements.
- Supervise care provided by LVNs and Home Aides in alignment with the established care plan.
- Ensure compliance with policies and regulatory standards during service delivery.
- Communicate updates and changes to the IDT and primary physician.
- Perform clinical duties as outlined in the competency checklist and adhere to established standards of care.
- Document patient interactions and care using electronic charting systems.
- Implement and uphold hospice policies, procedures, and regulatory requirements.
- Provide education and support to patients and their families.
- Train and orient new staff members.
- Participate in providing 24/7 coverage for patients and families as needed.
- Conduct admission assessments when required.
- Adhere to E-way standards in all job functions.
- Associate's Degree required; Bachelor's Degree preferred.
- A minimum of 12 months of nursing experience; 1-2 years in acute care preferred.
- Current California RN license.
- Valid California driver's license with proof of insurance.
- Certification as a hospice nurse is preferred.
- Pediatric experience is a plus.
- Spanish language proficiency is preferred.
- Proficient in computer skills, including word processing and basic office equipment.
- Ability to drive in various conditions and access homes with pets.
