LPN Care Coordinator

This position is primarily responsible for managing the care of patients, promoting effective education, self-management support, and timely health care delivery. The LPN Care Coordinator will serve as a liaison between residents, families, physicians, and other healthcare providers regarding healthcare issues.
Key Responsibilities:
Maintain a current knowledge of OSHA and state regulations and routinely monitor community compliance with regulations related to resident care.
Provide follow-up contact with patients to ensure compliance with recommendations.
Educate and involve patients to improve their health-patient engagement.
Assist patients to set goals, progress, and identify barriers when patients are not meeting treatment goals.
Manage many aspects of patient care, including referrals, hospitalizations, ER visits, ancillary testing, and other as directed to achieve optimum outcomes in a safe and cost-effective manner.
Develop a list of medical supply and community resources available to patients.
Operate department(s) within established budgetary guidelines.
Coordinate continuity of care with the patient and/or family to ensure compliance.
Provide clinical follow-up with patients, giving information and guidance to enhance patient-care team communication.
Maintain accurate and timely documentation.

Education and/or Experience:
Bachelor's degree (B.A.) from four-year college or university; or one to two years related experience and/or training; or equivalent combination of education and experience.
