SUMMARY:
This position is primarily responsible for managing the care of patients, promoting effective education, self-management support, and timely health care delivery by performing the following duties:

ESSENTIAL DUTIES AND RESPONSIBILITIES:
Core duties and responsibilities include the following. Other duties may be assigned.

Serve as liaison between resident, family, physician, and other health care providers regarding health care issues.
Maintains a current knowledge of OSHA and state regulations and routinely monitors community compliance with regulations related to resident care.
Provides follow-up contact with patients to ensure compliance with treatment plans and involves patients to improve their health-patient engagement.
Assists patients to set goals, progress and identify barriers when patients are not meeting treatment goals.
Manages many aspects of patient care: referrals, hospitalizations, ER visits, ancillary testing, and other as directed to achieve optimum outcomes in a safe and cost-effective manner.
Develops a list of medical supply and community resources available to department(s) within established budgetary guidelines.
Coordinates continuity of care with the patient and or family to ensure compliance and provides clinical follow-up with patients, giving information and guidance to enhance patient-care team communication.
Maintains accurate and timely documentation.

SUPERVISORY RESPONSIBILITIES:
This job has no supervisory responsibilities.

EDUCATION AND/OR EXPERIENCE:
Bachelor's degree (B.A.) from four-year college or university; or one to two years related experience and/or training; or equivalent combination of education and experience.
