Job Description

We are seeking a skilled RN Supervisor/Clinical Manager to join our team at American Care Partners, a leading home health care agency in the District of Columbia. As a Clinical Manager, you will be responsible for overseeing the delivery of high-quality patient care in the home setting.

Key Responsibilities:

  • Supervise and coordinate the care of patients with complex medical needs, including tracheotomy, in-home ventilator care, and G-tube management.
  • Develop and implement individualized care plans that meet the unique needs of each patient.
  • Collaborate with the interdisciplinary team to ensure seamless care transitions and optimal patient outcomes.
  • Conduct regular assessments and evaluations to ensure the highest level of patient satisfaction and care quality.

Requirements:
