Patient-Centered Medical Home Care Coordinator

The role of the Patient-Centered Medical Home (PCMH) Care Coordinator is to work collaboratively with the physicians, staff, and other healthcare professionals to actively facilitate healthcare delivery and promote care team communication for an assigned patient population, ensuring appropriate care is provided.
This position will be remotely based in the PeeDee area, with the requirement to round in clinics 1-2 days per month.
Key Responsibilities
- Identify patients who qualify for care coordination, including those not meeting clinical goals and quality measures, and arrange for follow-up services as appropriate.
- Utilize Epic registries and reports to identify patients and needs, and communicate effectively with patients, care teams, and providers to provide support for continuity of care.
- Schedule services and place referrals in accordance with patient needs, and follow up to track data.
- Maintain accurate records and communicate with providers and care team members to ensure seamless care coordination.
- Identify patient needs and barriers to care, and coordinate patients' contact with community resources.
Requirements
- Certified Medical Assistant (CMA) or Licensed Practical Nurse (LPN) with a high school diploma or equivalent.
- Completion of an accredited medical assisting program or recognized Licensed Practical Nursing program.
- Basic computer skills and ability to understand and implement detailed instructions.
- Ability to communicate effectively verbally and in writing.
Work Environment
- Remote work with occasional on-site visits to clinics.
- Ability to work independently under the supervision of the PCMH Clinical Manager.
