The MDS Coordinator plays a crucial role in ensuring the precision of resident evaluations and the enhancement of care plans. Key responsibilities of this position include:

  • Overseeing and facilitating the creation and completion of resident assessments (MDS) in compliance with current regulations and guidelines.
  • Guaranteeing the accurate and timely execution of the MDS process.
  • Collaborating with the interdisciplinary team to formulate personalized resident care plans.
  • Tracking and documenting resident progress and their responses to care plans.
  • Engaging in the Quality Assurance and Performance Improvement (QAPI) initiatives.
  • Communicating effectively with residents, families, and other healthcare professionals to ensure optimal care delivery.
  • Keeping abreast of MDS regulations and guidelines to maintain compliance.

Salary Range: Up to $84,000 annually.

Qualifications:

  • Current RN (Registered Nurse) license.
  • RAC-CT (Resident Assessment Coordinator-Certified) certification is mandatory.
  • In-depth knowledge of MDS 3.0, RAI process, and care planning.
  • Exceptional organizational, communication, and interpersonal skills.
  • Ability to work autonomously and collaboratively within a team environment.
  • Dedication to delivering high-quality care to residents.

This position is PRN and requires on-site presence.
