Effective adverse event reporting is crucial for identifying risks and enhancing safety. This role is responsible for overseeing the adverse event reporting system, providing daily support for the detection, processing, and reporting of adverse events (AEs). The position initiates, coordinates, and manages strategic initiatives aimed at reducing risks across the HMH Enterprise.

Key Responsibilities

A typical day for a Clinical Risk and Loss Prevention Specialist at Hackensack Meridian Health includes:

  • Working closely with Executive, Regional, Local, Nursing, and Clinical Leadership to cultivate a sustainable risk prevention culture that minimizes harm to patients and team members.
  • Partnering with Regional and local Campus Risk Management and Quality/Safety leaders to foster a culture of safety and reporting, promoting a Just Culture that encourages non-punitive reporting of adverse events.
  • Continuously enhancing the Hackensack Meridian Health Adverse Event Reporting platform to align with stakeholder needs, driving efficiency and high-quality data.
  • Utilizing the adverse event reporting system as a resource for leaders to gain insights into harm within their units.
  • Leading the governance framework to engage stakeholders in developing a reporting platform that meets the requirements of Enterprise Risk Management, Patient Safety and Quality, and Patient Experience.
  • Collaborating with data analytics to create de-identified dashboards for trend analysis.
  • Presenting findings at network and campus committees to raise awareness about learning opportunities from reporting.
  • Integrating machine learning, artificial intelligence, and automation into the existing system while monitoring for unintended effects and ensuring quality assurance.
  • Identifying barriers to reporting and developing solutions to enhance stakeholder involvement and improve the overall reporting culture.
  • Cooperating with site risk, patient safety/quality, and human resources leadership to address adverse events and grievances, utilizing analytical skills to recommend appropriate actions.
  • Promoting a safety culture where reporting is encouraged and the use of the Onelink adverse event reporting system is actively monitored.
  • Overseeing risk management software to enhance risk mitigation and safety culture across the network.
  • Providing regular updates to senior leadership regarding site activities.
  • Designing and implementing programs that promote a safe environment for all stakeholders.
  • Conducting statistical analyses for committee reviews as required.
  • Collaborating with stakeholders to streamline workflows for event reporting and analysis.
  • Creating educational and training initiatives for the platform to support clinical and operational risk reduction.
  • Envisioning and implementing enterprise-wide clinical risk guidelines and standards.
  • Developing Key Performance Indicators and Dashboards for Clinical Risk & Loss Prevention.
  • Offering litigation support to the legal team and external counsel.
  • Serving as a senior clinical risk advisor within the enterprise.
  • Leading or co-leading Patient Safety Organizations for HMH.
  • Performing additional duties and projects as assigned.
  • Adhering to organizational competencies and standards of behavior.

Qualifications

Required Education, Knowledge, Skills, and Abilities:

  • A minimum of 10 years of progressively responsible experience in the healthcare sector.
  • At least 3 years of experience in Healthcare Risk Management or Patient Safety.
  • Strong understanding of Project Management, Enterprise Risk Management, Process Review, Root Cause Analysis, and familiarity with regulatory requirements.

Preferred Education, Knowledge, Skills, and Abilities:

  • Bachelor's degree in Nursing, Business Management, or a related health field.

Preferred Licenses and Certifications:

  • Risk Management Certification.
