Position Overview: Under general supervision, the Clinical Nurse Specialist provides comprehensive nursing and health care services to individuals and families within an outpatient primary care setting, focusing on adult medicine. This role encompasses the management, delivery, and assessment of nursing care in designated areas of responsibility. The incumbent will facilitate both telephonic and in-person patient interactions, conduct physical examinations, perform laboratory tests, and deliver patient education. The Clinical Nurse Specialist responds to patient needs, ensuring that health care services align with the mission and values of Lowell Community Health Center.

Key Responsibilities:
  • Facilitates the delivery of care, including treatment, education, and counseling.
  • Identifies and interprets symptoms of patients' conditions, reporting findings to the appropriate health care provider.
  • Delivers direct nursing services to patients and their families, encompassing nursing assessments, interventions, referrals, and health education.
  • Evaluates patients' needs based on current functioning, strengths, previous health plans, and medications, while considering support systems and special needs.
  • Prepares patients for examinations by completing medical histories and reviewing current health status.
  • Measures vital signs, including blood pressure, weight, height, and temperature, as part of health assessments.
  • Conducts vision and hearing screenings when necessary.
  • Collects urine and sputum specimens as required.
  • Ensures accurate laboratory and radiology imaging reconciliation.
  • Administers immunizations and injections as appropriate.
  • Keeps clinical logs for central areas.
  • Develops a systematic approach for summarizing clinical and laboratory data for evaluation, planning, and quality assurance.
  • Assists in planning and implementing educational initiatives for patients and families to enhance awareness of medical conditions, reduce risk behaviors, and promote self-care.
  • Participates in in-service education programs, quality assurance initiatives, and risk management activities.
Qualifications:
  • Graduate of an accredited School of Nursing.
  • Registered Nurse License in the state of MA.
  • Current CPR/BLS certification.
  • Exceptional interpersonal and communication skills.
