Crisis Response Care Coordinator - Mobile Crisis Intervention Team

The Crisis Response Care Coordinator plays a vital role in advocating for, coordinating, and ensuring the continuity of evidence-based support for individuals and families in need through a collaborative approach involving triage, evaluation, planning, and effective communication. This position requires active participation as a member of a multidisciplinary treatment team and reports directly to the unit or department supervisor.

Qualifications:

  • A Bachelor's degree in a Human Services-related field along with a minimum of 1 year of experience in the mental health sector. For programs focused on child welfare, 2 years of experience with at-risk populations and a child welfare certification are mandatory.
  • Proficient in interpreting clinical documentation, procedures, or regulations, and capable of communicating effectively both verbally and in writing with diverse stakeholders.

Key Responsibilities:

  • Oversee the operations of the Mobile Crisis Referral Line and coordinate responses with the Mobile Crisis Intervention Team (MCIT).
  • Evaluate and prioritize incoming crisis calls to assess severity and assist with clinical staffing and MCIT response.
  • Facilitate access to community resources based on patient requirements and ensure all necessary documentation is completed.
  • Formulate service plans with specific goals and objectives while maintaining efficient workflow management.
  • Comply with established standards and meet the expectations and deadlines set by the supervisor and the agency.

Work Environment:

This role is situated in a dynamic, collaborative team environment that emphasizes critical thinking and decision-making. The position involves working in a climate-controlled office setting with interactions with clients, as well as potential travel within the region for in-home client services or onsite programs.
