We are seeking a skilled MSW Licensed Care Manager to join our Palliative Care Team at Salem Health.
Key Responsibilities:
- Conduct comprehensive psychosocial assessments for assigned patients and develop evidence-based interventions to support their care.
- Collaborate with the multi-disciplinary treatment team to coordinate care and services, ensuring seamless transitions and optimal patient outcomes.
- Participate in care conferences with palliative care physicians and other team members to discuss patient progress and develop treatment plans.
- Facilitate patient and family understanding, decision-making, and capacity to comply with medical recommendations.
- Document social work interventions and patient contacts in a clear and timely manner.
- Report mandated child and elder abuse/neglect as required by Oregon law.

Requirements:
- Master's Degree in Social Work from a CSWE accredited School of Social Work or Council for Accreditation for Counseling
- LMSW, LCSW or CSWA (pursuing LCSW) required by State of Oregon
- Two (2) years of experience in an inpatient and/or ambulatory environment preferred
