About This Role

We are seeking a skilled MSW Licensed Care Manager to join our Palliative Care Team at Salem Health.

Key Responsibilities:

  • Conduct comprehensive psychosocial assessments for assigned patients and develop evidence-based interventions to support their care.
  • Collaborate with the multi-disciplinary treatment team to coordinate care and services, ensuring seamless transitions and optimal patient outcomes.
  • Participate in care conferences with palliative care physicians and other team members to discuss patient progress and develop treatment plans.
  • Facilitate patient and family understanding, decision-making, and capacity to comply with medical recommendations.
  • Document social work interventions and patient contacts in a clear and timely manner.
  • Report mandated child and elder abuse/neglect as required by Oregon law.

Requirements:

  • Master's Degree in Social Work from a CSWE accredited School of Social Work or Council for Accreditation for Counseling
  • LMSW, LCSW or CSWA (pursuing LCSW) required by State of Oregon
  • Two (2) years of experience in an inpatient and/or ambulatory environment preferred
