Hospice Social Worker PRN

We are seeking a skilled and compassionate Hospice Social Worker PRN to join our team at Serenity Hospice. As a Hospice Social Worker PRN, you will play a vital role in providing high-quality patient care and support to patients and their families.
Responsibilities:- Conduct thorough psychosocial assessments of patients and families to identify their needs and develop individualized care plans.
- Provide counseling and support to patients and families to help them cope with stress and other challenges.
- Collaborate with healthcare teams to ensure seamless care coordination and communication.
- Document patient services and progress accurately and timely.
- Assist patients and families with applications for benefits and resources to meet their needs.
- Master's Degree in Social Work (MSW) from an accredited program.
- Current State Licensure in Social Work (LMSW or LCSW).
- Minimum one year of experience in healthcare or hospice settings.
- Valid driver's license and proof of insurance.
- Competitive hourly rate up to $43.
