Medical Social Worker

We are seeking a compassionate and skilled Medical Social Worker to join our Supportive Care team at Willamette Vital Health. As a Medical Social Worker, you will play a vital role in providing medical social services to palliative care patients, families, and caregivers.
Job Summary:The Medical Social Worker will provide comprehensive patient and family biopsychosocial-spiritual assessments, develop plans of care, and engage with patients and families to facilitate trust, rapport, and communication. The ideal candidate will have a strong background in social work, experience working with palliative care patients, and excellent communication and interpersonal skills.
Responsibilities:- Conduct comprehensive patient and family biopsychosocial-spiritual assessments
- Develop plans of care based on initial assessments and advance care planning
- Engage with patients and families to facilitate trust, rapport, and communication
- Provide brief supportive counseling, crisis intervention, and/or appropriate therapeutic supports as needed
- Act as liaison to promote discussion between patients, families, and healthcare teams
- Provide education and guidance to patients, families, and care team members
- Refer patients and families to appropriate supports and resources
- Advocate for patients and families to ensure respect for patient autonomy and maximize safety of care plans
- Masters in Social Work (MSW) required, LCSW preferred
- Two (2) years of demonstrated experience working with multigenerational family systems in a clinical capacity
- Two (2) years demonstrated experience working with life-threatening conditions, chronic disease, hospice, and/or end-of-life care, and related situations
- Demonstrated experience working with family resources and care systems
- Excellent verbal and written skills
- Must have reliable transportation, valid driver's license, and proof of auto insurance to travel to and from patients' homes
- Must be vaccinated against COVID-19
- Tuberculosis screening after hire required
