We are seeking a skilled Hospice Social Worker LCSW to join our team. As a key member of our interdisciplinary team, you will provide psychosocial assessments, counseling, and support to patients and their families. Your expertise will help us deliver exceptional care and make a meaningful difference in the lives of those we serve.
Responsibilities- Conduct thorough psychosocial assessments of patients and families
- Develop and implement individualized plans of care
- Provide counseling and support to patients and their families
- Collaborate with interdisciplinary team members to ensure seamless care
- Document patient services and progress
- Master's Degree in Social Work (MSW) or related field
- Current state licensure as a Licensed Clinical Social Worker (LCSW)
- Minimum one year of experience in healthcare or hospice setting
- CPR certification
- Strong communication and interpersonal skills
