Job Overview

PRIMARY FUNCTION:

The role involves supporting the Director of Social Work in managing the admissions, transfers, and discharges of residents at Menorah Park. This position provides both direct and indirect assistance to residents and their families, addressing their emotional, social, economic, and physical needs while fostering effective communication with staff, residents, and community resources.

KEY RESPONSIBILITIES:

  1. Facilitates the admissions, transfers, and discharges of residents to and from Menorah Park.
  2. Assists residents in navigating initial adjustments, interpersonal relationships, and various policy and financial concerns.
  3. Supports residents dealing with emotional challenges related to physical health issues, family dynamics, and transitions.
  4. Maintains ongoing communication with family members regarding resident status and concerns.
  5. Keeps accurate case records and prepares statistical reports as needed.
  6. Participates in relevant staff meetings and patient care discussions.
  7. Represents Menorah Park in community engagements and seminars as directed by the Executive Director.
  8. Stays informed about advancements in social work practices, particularly in gerontology, and applies new techniques as appropriate.
  9. Completes all mandatory in-service education and training sessions.
  10. Assists with administrative tasks as required.
  11. Performs additional duties as assigned by the Director of Social Work.

QUALIFICATIONS:

A High School Diploma or GED is preferred. The ideal candidate should possess the maturity, patience, and empathy necessary to effectively engage with elderly and disabled individuals and their families in various situations.
