About the Role

Nascentia Health is seeking a highly motivated and dedicated Registered Nurse to join our team as a Registered Nurse Residency Program. This is an exciting opportunity to work in a dynamic and innovative healthcare environment, providing holistic care to patients in their homes.

Key Responsibilities
  • Participate in the nurse residency program and educational opportunities offered through classroom, online, simulation, and in-home experience.
  • Complete all required assignments, online learning modules, and learning activities as assigned.
  • Learn how to ensure quality and safe delivery of home health services.
  • Develop a plan of care based on client assessment and OASIS evaluations.
  • Coordinate and collaborate with internal and external resources to meet patient needs.
  • Develop an in-depth understanding of community-based resources.
  • Maintain established productivity expectations.
  • Communicate client care issues to the interdisciplinary team.
  • Supervise home health aides in performing scope of practice appropriate tasks related to patient care and per care plan.
  • Comply with Nascentia Health policies and procedures and quality initiatives.
Requirements
  • Current New York State registered professional nurse license or may hold a Graduate Nurse License.
  • Must attain RN licensure within 90 days of hire.
  • Degree from an accredited school of nursing or college.
  • Current and valid driver's license and auto insurance, and a vehicle which can be used for work.
  • Self-directed and organized with the ability to work independently.
  • Basic computer knowledge with proficiency in Microsoft Office or other Word processing, spreadsheet, and database management.
  • Good oral and written communication skills.
  • Demonstrates motivation to learn a new role and is an active learner.
