Social Services Coordinator

As a Social Worker at Loretto, you will provide comprehensive social work services to residents in our skilled nursing facilities. This includes psychosocial evaluations, assessments for admission, patient rights and advocacy, and social casework, group work, and family counseling as appropriate. You will be a key member of the interdisciplinary care team, ensuring maximum quality of life for assigned residents.
Responsibilities
• Provide social work interventions for residents and families in times of crisis, deteriorating health, dealing with disabilities, and end-of-life situations.
• Educate residents, families, and caregivers on advanced directives and ensure they are aware of their rights.
• Serve as an advocate for residents in the absence of a caregiver or if the resident is unable to do so.
• Coordinate discharge planning, identifying resources and services for timely discharge, and ensuring complete, accurate, and timely documentation.
• Facilitate initial, annual, and as-needed resident and family meetings, ensuring communication with family members not in attendance.
• Provide effective orientation for new residents and families, reviewing resident rights and providing financial counseling as required.
• Assess resident financial situations, referring to the Finance Department as necessary, and provide Medicaid application support.
• Review significant events and occurrences related to family, finance, and psychosocial behavioral changes with the interdisciplinary team.
• Actively participate in the interdisciplinary team, sharing psychosocial information and contributing to the development of resident care plans.
• Maintain communication with team members to discuss progress and coordinate resident care efforts.
• Assess residents and families' adjustments to placement and environment, providing interventions as required.
• Provide timely social work services following departmental policies, procedures, and protocols, as well as regulatory guidelines.
• Comply with all third-party regulations, staying current in knowledge of changes to regulations and best practices in social work.
Qualifications
• Bachelor's degree in Social Work, Human Services, Sociology, Special Education, Rehabilitation Counseling, or Psychology.
• Minimum of one year of experience in geriatric care, long-term healthcare, or community services to the aging, in a direct service capacity.
• Proven assessment skills and ability to work in a team setting.
• Knowledge of regulatory requirements of DOH and all pertinent agencies.
• Outstanding interpersonal skills and ability to communicate effectively with clients, residents, and families from diverse backgrounds.
• Proficient in MS Office and able to learn new software rapidly.
