Job Summary

We are seeking a skilled Admissions Coordinator to join our team at St. Camillus Residential Health. As a key member of our admissions team, you will be responsible for coordinating inpatient referrals, gathering and reviewing medical, social, and financial information, and ensuring accurate and timely census activity.

Key Responsibilities
  • Coordinate inpatient referrals, including gathering and reviewing medical, social, and financial information
  • Ensure accurate and timely census activity
  • Maintain ongoing communication with the admissions team and nursing units
  • Coordinate the daily activity of the Intake Screener
Requirements
  • Current NYS RN license or Current NYS LPN License required
  • Valid CPR AED/BLS Certificate
  • Valid NYS driver's license and a reliable vehicle with insurance
  • Experience in care coordination responsibilities
  • Knowledge of third-party payor regulations, including Medicare and Medicaid
  • Excellent assessment and strong verbal and written communication skills
  • Basic computer skills
