LPN Care Coordinator

Key Responsibilities
- Develop and implement safe, economical, and efficient care plans.
- Coordinate the care received by residents, ensuring seamless transitions and effective communication.
- Provide direct care to residents, including medication administration and wound care.
- Implement all phases of the nursing process, including assessment, planning, implementation, and evaluation.
- Document all information related to resident care, maintaining accurate and up-to-date records.
- Recognize changes in resident status and report to appropriate personnel, ensuring timely interventions and support.
- Act as a liaison between residents, families, and healthcare providers, facilitating effective communication and collaboration.
- Maintain timely physician orders, ensuring that all necessary documentation is completed and up-to-date.
- Collaborate with other healthcare professionals to ensure comprehensive and coordinated care.
- Participate in quality improvement initiatives, contributing to the development and implementation of best practices.
Requirements
- Active LPN license in the state of New York.
- Minimum of 1 year of experience in a long-term care or healthcare setting.
- Strong communication and interpersonal skills, with the ability to work effectively with residents, families, and healthcare providers.
- Ability to work independently and efficiently in a fast-paced environment, prioritizing tasks and managing time effectively.
- Proficient in electronic medical records systems, with experience using software to document resident care and communicate with healthcare teams.
- Excellent organizational and time-management skills, with the ability to prioritize tasks and manage multiple responsibilities.
- Demonstrated empathy, compassion, and patience towards residents and their families, with a commitment to providing high-quality care and support.
