Job SummaryMenorah Park, a leading healthcare provider in Syracuse, New York, is seeking a skilled LPN Care Coordinator to oversee the care of our residents. Key Responsibilities
  • Develop and implement safe, economical, and efficient care plans.
  • Coordinate the care received by residents, ensuring seamless transitions and effective communication.
  • Provide direct care to residents, including medication administration and wound care.
  • Implement all phases of the nursing process, including assessment, planning, implementation, and evaluation.
  • Document all information related to resident care, maintaining accurate and up-to-date records.
  • Recognize changes in resident status and report to appropriate personnel, ensuring timely interventions and support.
  • Act as a liaison between residents, families, and healthcare providers, facilitating effective communication and collaboration.
  • Maintain timely physician orders, ensuring that all necessary documentation is completed and up-to-date.
  • Collaborate with other healthcare professionals to ensure comprehensive and coordinated care.
  • Participate in quality improvement initiatives, contributing to the development and implementation of best practices.

Requirements
  • Active LPN license in the state of New York.
  • Minimum of 1 year of experience in a long-term care or healthcare setting.
  • Strong communication and interpersonal skills, with the ability to work effectively with residents, families, and healthcare providers.
  • Ability to work independently and efficiently in a fast-paced environment, prioritizing tasks and managing time effectively.
  • Proficient in electronic medical records systems, with experience using software to document resident care and communicate with healthcare teams.
  • Excellent organizational and time-management skills, with the ability to prioritize tasks and manage multiple responsibilities.
  • Demonstrated empathy, compassion, and patience towards residents and their families, with a commitment to providing high-quality care and support.
