We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our Home Health team at Providence. Key Responsibilities:- Provide skilled nursing care to patients in their homes, including wound care, medication management, and vital sign monitoring.
- Conduct thorough assessments of patients' physical and emotional needs, developing and implementing individualized care plans to meet those needs.
- Collaborate with patients, families, and healthcare providers to ensure seamless care transitions and optimal patient outcomes.
- Document patient progress and care provided, maintaining accurate and timely records.
- Graduate of an accredited LPN/LVN school of nursing.
- Current Oregon Licensed Practical Nurse (LPN) licensure.
- One year of experience in nursing, preferably in home health or a related field.
