We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our Home Health team at Providence. As a Home Health Nurse, you will provide high-quality patient care in the comfort of their own homes, working collaboratively with our interdisciplinary team to deliver comprehensive care.

Key Responsibilities:
  • Provide skilled nursing care to patients in their homes, including wound care, medication management, and vital sign monitoring.
  • Conduct thorough assessments of patients' physical and emotional needs, developing and implementing individualized care plans to meet those needs.
  • Collaborate with patients, families, and healthcare providers to ensure seamless care transitions and optimal patient outcomes.
  • Document patient progress and care provided, maintaining accurate and timely records.
Requirements:
  • Graduate of an accredited LPN/LVN school of nursing.
  • Current Oregon Licensed Practical Nurse (LPN) licensure.
  • One year of experience in nursing, preferably in home health or a related field.
