We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our Home Health team. As an LPN, you will provide high-quality patient care in the comfort of their own homes, working collaboratively with our interdisciplinary team to deliver comprehensive care.
Key Responsibilities- Conduct thorough assessments of patients' physical and emotional needs
- Develop and implement individualized care plans to meet patients' unique needs
- Administer medications and treatments as prescribed by physicians
- Provide education and support to patients and their families on disease management and self-care
- Collaborate with healthcare team members to ensure seamless care transitions
- Graduate of an accredited LPN/LVN school of nursing
- Current LPN licensure in Oregon (and Washington, if practicing in that state)
- Basic Life Support (BLS) certification
- Valid driver's license and reliable transportation
- Experience in home health, hospice, or public health
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
