About the Role

We are seeking a compassionate and skilled LPN to join our Home Health team at Providence. As a LPN, you will provide high-quality patient care in the comfort of their own homes, working collaboratively with our interdisciplinary team to deliver comprehensive care.

Responsibilities
  • Provide skilled nursing care to patients in their homes, including wound care, medication management, and vital sign monitoring.
  • Conduct thorough assessments of patients' physical and emotional needs, developing and implementing individualized care plans to meet those needs.
  • Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive care.
  • Communicate effectively with patients, families, and caregivers to ensure seamless care transitions and address any concerns or questions.
  • Document patient care and progress in a timely and accurate manner, adhering to regulatory requirements and organizational standards.
Requirements
  • Graduate of an accredited LPN/LVN school of nursing.
  • Current licensure as an LPN in the state of Oregon (or Washington, if practicing in that state).
  • One year of experience in nursing, preferably in home health or a related setting.
  • Ability to work independently and as part of a team, with excellent communication and interpersonal skills.
  • Strong problem-solving and critical thinking skills, with the ability to prioritize tasks and manage time effectively.
