Key Responsibilities
The EHE Retention Specialist plays a crucial role in aligning AHF's mission with practical experience in healthcare navigation. This position involves engaging with newly diagnosed clients, those who have disengaged from care, and current patients, offering guidance through the complexities of the healthcare system. Your empathy and interpersonal skills will foster trust and comfort among our clients.
In collaboration with the Care Management team and Healthcare Center staff, the Retention Specialist will ensure that each client receives tailored support and access to necessary resources. This includes providing education and enhancing health literacy.
By leveraging your expertise, you will meet clients at their current stage, identify their strengths, and assist them in achieving their health goals. Your collaborative approach will embody the principles of the Ryan White Care Act, serving as a safety net for uninsured individuals affected by HIV/AIDS. Your interactions with patients will directly contribute to AHF's success in fulfilling the objectives of our Ending HIV Epidemic contract, as well as the broader measures set by the HIV/AIDS Bureau (HAB).
Qualifications
We seek candidates who are bilingual in English and Spanish, reflecting our commitment to serving a diverse clientele. A minimum of one year of case management experience in a healthcare environment is required. Experience as a retention specialist and a background in mental health services for vulnerable populations are advantageous. A strong foundation in customer service is essential for this role.
