Job Summary

The Illumination Foundation is seeking a skilled Case Manager to join our team. As a Case Manager, you will be responsible for providing housing advocacy and specialized support to clients.

Key Responsibilities
  • Client Support
    • Develop and implement holistic treatment plans to address clients' needs
    • Provide assistance and referrals to clients to meet their psycho-social and daily needs
    • Collaborate with clients to set and achieve weekly goals and completion of action steps
  • Case Management
    • Complete Client Outcome Matrix and utilize KIPU and HMIS to track and maintain accurate case notes and client and program data
    • Collaborate with the Case Management Team to discuss the effectiveness of programs and client progress
  • Crisis Intervention and Counseling
    • Perform crisis intervention as necessary
    • Counsel clients with confronting issues adversely affecting their lives
  • Administrative Tasks
    • Establish and maintain confidential case files for participants
    • Prepare required statistical reports for program management and evaluation
Requirements
  • Education and Experience
    • Bachelor's degree in Social Services, Healthcare, or related field
    • Homeless services, case management, mental health, permanent supportive housing experience
  • Skills and Qualifications
    • Current valid CA driver's license
    • Familiarity with VI-SPDAT
    • Experience in non-profit housing and/or housing for people with disabilities and chronic health conditions
  • Preferred Qualifications
    • At least 1 year of experience working with at-risk/homeless individuals
    • Bilingual in English and Spanish
    • Proficiency in Microsoft Office Suite (Outlook, Word, Excel, Teams)
