Community Health Action of Staten Island (CHASI) is seeking a highly motivated and experienced HIV Client Services Coordinator to join our team. The successful candidate will be responsible for coordinating care services for individuals living with HIV, ensuring timely access to medical care, and promoting optimal treatment adherence.
Key Responsibilities:- Lead case finding activities and client recruitment efforts
- Facilitate the enrollment process through comprehensive assessments and service planning activities
- Work closely with the Program Director to ensure monthly objectives are achieved
- Supervise and coach Patient Navigators to ensure high-quality care coordination
- Collaborate with the medical team to manage program recruitment, enrollment, and ongoing patient navigation needs
- Conduct case conferences with providers and care coordination team members
- Track pertinent health outcome data and provide program performance reports
- Perform other duties as assigned
- At least three years of direct service experience in HIV care management
- At least one year of experience supervising two or more employees
- Bachelor's degree preferred
- Bilingual (English/Spanish) preferred
- Experience using Microsoft products such as Outlook, TEAMS, Word, Excel
- Active listening and feedback
- Clear and accurate communication
- Collaboration and teamwork
- Problem-solving and adaptability
- Leadership and supervision
