Care Coordinator II Job Description

Children's Aid is seeking a highly skilled Care Coordinator II to join our team. As a Care Coordinator II, you will play a critical role in ensuring that families receive the medical and behavioral health care services they need to promote optimal health.

Key Responsibilities:

  • Provide comprehensive care management services to a caseload of up to 35 clients, including assessment, planning, and coordination of services.
  • Develop and implement individualized plans of care that address the unique needs of each client.
  • Collaborate with healthcare providers, social services agencies, and community organizations to ensure seamless care coordination.
  • Conduct field visits and home visits as needed to assess client needs and provide support.
  • Document all activities, including progress notes, in a timely and accurate manner.
  • Participate in program-wide activities, including meetings and training sessions.

Requirements:

  • Minimum Bachelor's Degree required with 2 years of related experience.
  • Experience in serving children and families with chronic medical conditions, developmental disabilities, and mental/behavioral health diagnoses.
  • Excellent organizational and time management skills.
  • Strong verbal and written communication skills.
  • Ability to engage disenfranchised populations.

Working Conditions:

This is a full-time position that requires flexibility in scheduling, including evenings and weekends. The incumbent is expected to work a schedule that facilitates effective collaboration with the team and ensures the smooth operation of the department's processes.
