Position Overview

Kids Above All is looking for a dedicated Case Manager to support our mission in providing essential services for youth in a group home environment.

Role Summary: The Case Manager will be responsible for coordinating and delivering a range of services and activities tailored for young individuals residing in a group home. This role encompasses managing appointments related to health, education, and employment, as well as facilitating family visits. The Case Manager will serve as a key link between the organization and various external agencies involved in the clients' care, such as child welfare and judicial systems. Collaboration with team members and stakeholders is vital to effectively implement a permanency plan for each client.

Key Responsibilities:

  • Manage a caseload of clients, focusing on addressing their environmental and psychosocial needs.
  • Provide counseling and casework services to youth and their families regarding their permanency plans.
  • Organize and oversee family visitations with parents and guardians.
  • Maintain accurate records and enter client information into the management system.
  • Engage in case discussions with supervisors and team members to ensure comprehensive care.
  • Fulfill various case management tasks, including preparation for court proceedings and administrative reviews.
  • Stay informed about job-related competencies and client-specific needs.
  • Contribute to Continuous Quality Improvement initiatives.
  • Attend team meetings and participate in case evaluations and planning.
  • Work collaboratively with medical professionals during assessments and consultations.
  • Recognize and leverage community resources to support the needs of children and families.

Qualifications:

  • A Bachelor's degree in Human Services or a related discipline, with one to three years of relevant experience.
  • Successful completion of all required examinations and certifications.
  • Possession of a valid driver's license and an insured vehicle that meets safety standards.
  • Strong awareness of the needs of children and the ability to engage effectively with adults.
  • Proficient organizational skills and the ability to prepare necessary reports.
  • Willingness to work flexible hours, including evenings and weekends, as needed.
  • Basic computer proficiency, particularly in Microsoft Word.

Compensation: Salary range is $47,400 to $50,900.
