Job Title: QIDP Coordinator

Job Summary:

We are seeking a highly skilled and compassionate QIDP Coordinator to join our team at Reliance Home Health Caregivers. As a QIDP Coordinator, you will be responsible for providing exceptional care and support to our clients, while also ensuring that our services meet the highest standards of quality and excellence.

Key Responsibilities:

  • Evaluate client performance and provide feedback to ensure growth and improvement in specific skills.
  • Develop and implement individualized plans to meet client needs and goals.
  • Conduct regular staff meetings to coordinate service delivery and ensure effective communication.
  • Train new staff members and conduct in-services to ensure compliance with policies and procedures.
  • Participate in meetings and contribute to problem-solving and decision-making.
  • Write and review individual program plans to ensure accuracy and effectiveness.
  • Monitor and document client progress and outcomes.
  • Reinforce positive behavior and social patterns to improve client independence.
  • Exchange information with diagnostic staff, colleagues, and supervisors to maintain records and plan services.
  • Formulate client objectives and evaluate performance to ensure personal growth and improvement.
  • Inspect and monitor clients for appropriateness and take corrective action if necessary.
  • Substitute for absent coworkers and ensure the day-to-day functioning of the unit.
  • Complete required reports and forms to maintain compliance with agency and state requirements.

Requirements:

  • Four-year college degree and one year of directly related work experience.
  • Valid driver's license and current vehicle insurance.
  • Strong interpersonal skills and ability to work effectively with clients, staff, and outside individuals.
  • Proficient in word processing, database management, and spreadsheet software.
  • Ability to work efficiently and effectively in a fast-paced environment.

Education:

  • Bachelor's degree (required).

  • QIDP: 1 year (preferred).
