Job Description

At Center For Family Life in Sunset Park Inc, we are seeking a highly skilled and compassionate Bilingual Case Planner to join our team. As a key member of our staff, you will be responsible for providing family-centered services to families in our community.

Key Responsibilities:

  • Collaborate with families to develop and implement individualized service plans
  • Provide family, individual, and group counseling services
  • Conduct home and community visits to assess family needs and provide support
  • Act as a liaison between families, social service agencies, and community providers
  • Develop and maintain relationships with community partners to ensure seamless service delivery

Requirements:

  • Bachelor's Degree in Social Work or related field, or Master's Degree in Social Work (MSW) or equivalent mental health professional
  • 2 years of experience working with families, preferably in a social services setting
  • Strong communication and interpersonal skills
  • Ability to work effectively in a fast-paced environment
  • Bilingual: Spanish or Chinese/Mandarin, required

Working Conditions:

  • Share an office with one other program staff
  • Access to an individual desk and computer
  • May also attend various meetings with different personnel and supervisors
  • Required to travel to conduct home and community visits

Scope of Responsibility:

The Bilingual Case Planner is responsible for the planning and management of families in preventative programs, including yearly group, home, and community visits, process treatment plans, statistics, and all required documentation.

This is a full-time non-exempt position at 35 hours per week. The salary range is $45,000 to $53,000 per year.
