About the Role

We are seeking a highly skilled and compassionate Access and Enrollment Specialist - RN to join our team at Amorem. As a key member of our interdisciplinary group, you will play a vital role in ensuring patients and their families receive timely access to our services and support.

Key Responsibilities
  • Respond to referrals with a sense of urgency, completing evaluations and assessments of patient needs, coordinating care, and creating individualized plans of care.
  • Provide education on hospice and palliative care services, evaluating patient and family needs, and determining eligibility for care.
  • Admit patients, obtain informed consent, and complete related documentation in accordance with Amorem policies and procedures.
  • Communicate patient and family information to the interdisciplinary group, developing individualized plans of care and coordinating care.
  • Rotate 24-hour on-call services, providing duty as a primary nurse as needed, and completing documentation within designated hours.
Requirements
  • Current NC license as a registered nurse.
  • Minimum 2-3 years of nursing experience, with active patient contact or completion of a nursing refresher course within the past 3 years.
  • CPR Certification.
Physical Demands
  • Visual acuity to read written documentation and computer tasks.
  • Speech and hearing skills necessary for telephone interaction and face-to-face contact.
  • Ability to sit extended periods, perform administrative tasks, and stand, bend, and lift an average of 30 pounds.
Special Requirements
  • Must have an automobile with required insurance coverage and a state driver's license.
