As a full-time Access and Enrollment RN, you will leverage technology to improve quality of care, provide education to patients and families, maximize hospice medical skills, and approach patients and families holistically. You will respond to all referrals with a sense of urgency, evaluate patients, and collaborate with a dynamic interdisciplinary team to admit to appropriate programs while developing individualized plans of care.
